A. Registration Period & Fees
a. Early Bird Registration: August 1, 2012 to December 31, 2012 @ P2,500
b. Regular Registration: January 1, 2013 to February 28, 2013 @ P3,500
c. Late Registration: March 1, 2013 to March 7, 2013 @ P5,000
d. “NO REGISTRATION after the March 7 Deadline and NO ON-SITE REGISTRATION will be ACCEPTED”

B. Registration Procedure
a. Fill up REGISTRATION FORM & send preferably on-line or by mail or courier.
b. Pay Registration Fee by depositing amount in any of the depository banks:
1. Bank of the Philippine Islands, Central Market Branch, Iloilo City

c. Send accomplished REGISTRATION FORM together with proof of payment thru:

e. Submission of accomplished REGISTRATION FORM without proof of payment does
not make the delegate registered.
